Common questions

After your hospital stay.

Does this replace my primary care doctor?

No. This is focused transitional care while you reconnect with your primary care clinician and specialists. Keep arranging the follow-up recommended by your discharge team.

How soon should I be seen?

Follow the timing in your hospital discharge instructions. Tell our team what was recommended so we can confirm whether we can meet that timeframe. An appointment request is not a confirmed visit.

Can my follow-up be done by telehealth?

Some visits may be appropriate for telehealth. Symptoms, clinical needs, location requirements, and availability determine the right setting. A hands-on examination or procedure requires an in-person visit.

What if I feel worse before my appointment?

Follow your discharge return precautions. For a medical emergency, call 911 or go to the nearest emergency department. Do not wait for a routine appointment or scheduling response.

What if I was discharged after surgery?

Continue following your surgeon’s instructions and keep your surgical follow-up appointments. Contact our team to discuss whether additional outpatient medical follow-up is appropriate.
