What we can discuss

Understand what you’re taking—and why.

Bring your discharge prescriptions and usual medication list so your clinician can review your treatment in context. Any change depends on your clinical evaluation.

01

Review your medication list

Discuss your usual medications, new prescriptions, supplements, allergies, and questions about duplicate or changed instructions.

02

Follow your response to treatment

Review how you are doing and concerns about the plan you received at discharge.

03

Connect short-term and ongoing care

Discuss appropriate blood pressure, glucose, or non-controlled pain follow-up, then reconnect with your primary care clinician or specialist for ongoing management.

Before your appointment

Bring the details
of your hospital or ER visit.

Use the approved clinical channel our team provides for records. Missing paperwork is not an automatic barrier; contact our team to discuss what is available.

  • Medication bottles or an accurate medication and supplement list
  • Known allergies and discharge instructions
  • Questions about treatment and any readings requested by your care team
Common questions

Before your follow-up.

Are refills guaranteed?

No prescription, refill, or specific treatment is guaranteed. Follow Up Dr. does not refill opioids or narcotics, benzodiazepines, or stimulants.

What if I have a medication concern before my visit?

Contact a treating clinician promptly and follow your discharge instructions. For severe symptoms or a suspected emergency, seek emergency care rather than waiting for an appointment.

This is focused post-discharge care. Long-term medication management belongs with your usual primary care clinician or specialist.
Your next step

You’re home.
Let’s talk about what comes next.

Request a follow-up appointment through AM&A. Our team will confirm availability and the appropriate visit type.

A request is not a confirmed appointment. Follow the timing and return precautions in your discharge instructions.
